Hair loss stable for 8+ years – is it still possible to regrow, or should I just accept it?

Hi everyone,
I’m a 30s male and have been dealing with hair thinning for many years now. I recently shaved my head and noticed that despite the thinning on the crown and mid-scalp, there are still a lot of miniaturized hairs visible.

I’m attaching photos of my scalp both shaved and unshaved. This has been my baseline for about 8 years – very little progression.

My questions are:
Do the black dots on my scalp mean that they are still alive? Why arent they growing?

Is it still worth starting treatment (Finasteride, Minoxidil, Microneedling)?
Or does long-term stability mean the hair loss has “burned out” and I should just accept it?

since your hair loss has been stable, trying treatments like fin, min, and microneedling can be worthwhile. The black dots are miniaturized hairs, which might respond well to treatments. Check out the Beginner’s Guide for more info.

Thanks for your info! One more question: Based on my photos and the amount of miniaturized hairs I still have, would you recommend starting with finasteride alone for 12 months to see how well I respond, or is it better to add minoxidil right from the beginning? I’m trying to figure out whether a ‘fin-only first’ approach makes sense in my case, or if late minoxidil would give me significantly worse long-term results.

fin+min is better than just fin, no hair follicles don’t die overnight or within 1-3 years. sometimes they live for well past a decade, so lose no time and hop on min if you are prescribed min in addition to the fin. see a doctor before making medical decisions as always and keep in mind that this is not a medical advice.

Try oral dutasteride + topical minoxidil with tretinoin (you can find this compound online many places).

Dutasteride is simply better than finasteride with only a marginally increased side effect profile
