Diffuse thinner asking for help. Density patches, shedding, seb derm and itching

Hi guys, sorry this is going to be a bit long but I wanted to ask for help and opinions. I’m a diffuse thinner diagnosed with AGA by a dermatologist, also seb derm. The thing is recently I’ve noticed a couple of things that have made me wonder if I have something else because I don’t think derma looked at my scalp thoroughly.

First, I wanted to say that I’ve been on treatment since nov 2024. First 7 months topical minox, then in june 2025 I started 1mg fin and switched to 2mg oral min. My results have been mostly maintenance and in feb 2026 I think I noticed slight density improvement but not sure because I didn’t make progress photos. In late march I upped my oral min dose to 4mg to boost regrowth and since then hair has only gotten worse, I started shedding around 150 hairs a day 2 weeks later and have stayed the same, altough since june it’s been more like 100-120 hairs. It’s probably a shed but almost 4 months after I’m worried about it for a couple of reasons.

In feb 2026 my seb derm worsened and I had a lot of dandruff, so a couple of weeks later I started ketaconazole 2%, unfortunately it didn’t work for me, I used it correctly twice a week for a month then once a week after, but in april my scalp had only gotten even worse, yellow dandruff and itching appeared so I stopped using it. In may a doctor prescribed me ciclopirox olamina and It has worked to remove yellow dandruff, most of the normal white ones and improved the itch, but is a medical champoo so I can’t keep using it after the one month treatment.

The problem is the place where I still have visible white dandruff and most of the itch is the back of my head and also in the 3 months I’ve been shedding more than normal I have lost a lot of hair from the back and a bit of the sides. So after a haircut I decided to take some photos and I see clear patches with less density and sure it could be a huge shed but I’m worried of the possibility I have some type of alopecia areata or retrogade alopecia on top of the AGA. Also I did blood test in may just to be sure and everything came normal except a severe deficit in vit D and I’ve been fixing it with suplement since then.

Sounds like your shedding might be linked to the seb derm and vitamin D deficiency. Keep up with the vitamin D supplementation. Try ketoconazole or ciclopirox at a consistent frequency to control dandruff and irritation. The increase in oral min dosage could cause initial shedding, so watch for improvements over the next few months.
