Conflicting Trichologist Advice: Dosing, Side Effects, and Mesotherapy Concerns

Hi, sorry for the long post but I’m a bit conflicted right now. I’ve been worried about my hairline receding for a few years. It’s been slow, but it’s now noticeable, so I decided to finally seek help while it’s still early and I still have hair. I went to see trichology specialists, and I’ve been pretty disappointed.

To clarify, I’m nowhere near bald yet — my hairline has receded, but my crown still looks fine but I know it will be the next.

At the first clinic, which belonged to a big chain, the visit felt completely streamlined and scripted. He explained what dutasteride does, what minoxidil does, and then prescribed 5mg minoxidil and 0.5mg dutasteride for only 3 months(so I had to pay a visit again I assume), plus many sesions of mesotherapy. I was taken aback — he wanted me to come in every 2 weeks for 2 months, then monthly after that. Not to mention the price, and the fact that the procedure is basically injecting the same thing I’m already taking with the pills? why?

So yesterday I went for a second opinion at a smaller clinic in my hometown that does a bit of everything, and it was worse. No mention of side effects at all — she just said “if you get any, stop taking the pills.” She immediately scheduled a mesotherapy session in 2 weeks and prescribed 5mg minoxidil and 0.25mg dutasteride, 100 euros visit by the way. I’m a bit upset, these visits aren’t cheap, and it feels like they just want to sell me mesotherapy and don’t care much about the pills dosis and adverse effects.

Since that first visit I’ve done some research on my own, and I found a study (https://repositorio.unesp.br/entities/publication/b4067e5f-bb7d-494e-8590-deefad621084) showing there isn’t much added benefit between 2.5mg and 5mg oral minoxidil, while side effects like hypertrichosis and edema do seem to increase at the higher dose. That’s made me hesitant about the 5mg I was prescribed, especially since I consider myself an early case.

So a few things I’d love input on:

  1. Dosing — has anyone here started at 2.5mg minoxidil instead of 5mg, and did it work fine for you? Also curious about dutasteride — is 0.25mg considered a “real” dose or too low to do much? And how bad does the body hair growth actually get?

  2. What’s actually worked for you — what combo/doses got you good results, and how long did it take before you saw a real difference?

  3. Mesotherapy — does it really add benefit compared to just taking pills? Or is it mostly a moneymaker? Both clinics pushed it hard, it’s expensive and looked like their main source of income, so I’m skeptical, but wanted to check if anyone’s experience says otherwise.

I’m not a doctor, so I’m not sure whether I’m just being a drama queen or not. That’s why I decided to ask people with real world experience, and to be honest I don’t feel like paying for a third visit either TT

  1. Starting with 2.5mg oral min can work for some and helps reduce side effects. Keep an eye on any symptoms like edema or unwanted hair. 0.25mg dut is a solid dose for hair loss since it blocks more DHT than fin.

  2. Many stick with ‘the big 4’ (min, fin/dut, keto shampoo, microneedling) which are most effective for AGA. Results can take months, often up to a year.

  3. Mesotherapy is widely considered less effective and often questioned for its cost vs. benefit compared to meds. Clinics might promote it more than it’s worth scientifically.
