28M with Diffuse Thinning and Donor Area Affected, Dutasteride Not Working, Advice on Next Steps?

I’m 28 and started thinning about 4 years ago. It’s mostly diffuse with slight temple recession.

I tried fin 1mg for 9 months (no effect, DHT even went up) and have been on dut 0.5mg for 8 months (~70% lower DHT but no noticeable impact on hair). I also used topical min for 2 years, now 2.5mg oral for 1 year, again with no effect. My blood work has always been normal with no deficiencies.

What worries me is that I see miniaturization not only on top but also around the ears and in the back. The sides were always a bit thinner, but now I can consistently find wiry, thin hairs there too.

How can I best figure out what kind of hair loss this is, and what should I do next now that Dut seems not to be working?

Diffuse thinning can be tricky, and if dut isn’t working, it might be worth exploring other conditions like chronic TE. Min can sometimes help, even alongside dut, and oral min might be stronger than topical. Check for other possible causes of thinning and maybe give microneedling a shot. Thank you for the reply! Oral Min has already been part of my stack for about a year now. I plan to add microneedling soon, but it seems dut + min has not made any change for me so far.
I have seb derm but I would believe I am keeping it in check succesfully with Ketoconazol and Nizoral.
As pointed out, my bloodwork has never indicated any other issue. I am still lost on how to understand the exact condition responsible for my hair loss. Is it just an aggressive version of AGA (retrogade AGA or DUPA)? All in all I want to make sure that I am not missing out on a much more sensible treatment option for my specific case..

2.5 duta will stop progressoin of any AGA. If you’re still losing hair, then it’s probably not AGA. Your doctor should be diagnose you

My doctor seems not to be familiar with Dupa or retrogade alopecia. Also, Dutasteride has lowered my DHT from 350 to 70 ng/dl and I already experience noticable side effects. I am not sure whether it would be responsible to increase to such a high dosage. What is the likelihood that Dutasteride begins working at 2,5mg if I see no effect at 0,5mg?

Depends on the condition you have, if you have AGA and 0.5mg duta isn’t working, then you probably don’t have AGA. 2.5mg just gives you more regrowth. You should get a diagnosis from a dermatologist that knows his shit
