23M, 12 Months After 4500 Graft Transplant, No Minoxidil/Finasteride - Next Steps?

Hello everyone,

I had a hair transplant on 15 March 2025 in Turkey (around 4500 grafts).

I am now almost 12 months post-op. The transplanted hair grew, but overall density still looks thin and my scalp is visible under light.

Current situation:

  • No finasteride

  • No minoxidil

  • Only taking biotin daily

  • No medical treatment before or after transplant

  • Age: 23

My questions:

  1. Is this density normal at ~12 months?

  2. Could ongoing native hair loss explain the thinning?

  3. Is starting finasteride/minoxidil still useful this late after transplant?

  4. At what point should someone consider a second procedure?

It’s pretty normal for hair to look less dense at 12 months if you’re not using fin or min. Ongoing hair loss can definitely thin your look. Starting fin or min now can help improve and keep your density. Thinking about a second transplant isn’t uncommon, but starting treatment now is key.

I am using minoxidil, I was thinking if it stops would be worsen?
